Over 25 million Americans shoulder the crushing responsibility of caring for ill family members, yet their experiences remain largely invisible within the healthcare system. Drawing from his dual perspective as a medical educator and rabbi who cared for his wife following her 1986 brain aneurysm rupture, the author employs autoethnographic methodology to examine the ethical and practical challenges faced by spousal caregivers. Through the biblical metaphor of "Mrs. Job," the analysis reveals how technological advances have paradoxically burdened families by shifting complex medical responsibilities from hospitals to untrained caregivers in home settings. The research identifies critical tensions between traditional Jewish bioethical principles emphasizing life preservation and modern healthcare realities constrained by limited resources and quality-of-life considerations. Caregivers experience significantly elevated rates of psychological distress and health complications, yet healthcare curricula and support systems remain inadequate to address their needs. The findings demonstrate the urgent need for comprehensive policy reforms that both recognize caregivers' central role in healthcare delivery and address the moral complexities of medical decision-making in resource-constrained environments.
